Student Application Information

Faith Family Academy is currently accepting applications for the 2009-2010 school year, and we will be accepting applications for the 2010-2011 school year in February. Should you want to complete an application, please print the application below for each student and bring the key documents listed below to our registrars office located on each campus. Incomplete applications will not be processed.

This application is for the 2009-2010 school year. If you are unable to print out the application, we will be more than glad to provide you with one, and a place to fill it out on campus. Once you provide the registrar with the information requested, depending on the grade level that your student is enrolling, you may be asked to fill out additional information.

Items to bring for enrollment:

  • Completed application
  • $10.00 Non-refundable application fee
  • Birth Certificate
  • Shot Record
  • Social Security Card
  • Proof of Residence (Water bill, Etc.)
  • Latest report card
  • Transcript (students grades 9-12)
  • TAKs Scores (students grades 3-12)
  • Grade placement committee paperwork (for students who failed TAKS in grades 3, 5 & 8)
  • LPAC Information (for LEP students)
  • Current ARD paperwork ( for Special Ed students)
  • ID of person/parent enrolling student

 

It is the policy of Faith Family Academy Charter School to comply with non-discrimination provisions of all federal and state laws. FFA admits students without regard to race, religion, color, gender, age, national origin, ethnicity, disability, academic, artistic, athletic ability, marital status, political belief, limited English proficiency or the district the child would otherwise attend. FFA will deny admission to students with documented histories of a criminal offense, juvenile court adjudication, or discipline problems under TEC Chapter 37, Subchapter A.  If anyone believes that they have been discriminated against, they should write to the Secretary of Education, Washington D.C. 20250.

Child Find - a  Free Appropriate Public Education (FAPE) is provided to all individuals with disabilities, ages 4-21, who qualify for Special Education services. With the assistance of the Texas Education Agency, ESCs, and the Early Childhood Intervention (ECI) program, LEAs must ensure that early intervention services are made available for those children, ages 0-3, with an identified need.
